The VA recently took the Disability Benefits Questionnaires for private professionals to use off of their website. I think it would likely be best for private professionals to address this evidence inside a standard report (ex: a psychological evaluation) rather than on the DBQ form. However, for reference you may be interested in these. Please keep in mind that I don’t intend to update these over time and they are not necessarily the current forms. They are meant for professionals to be able to address the correct information in their examinations.
